ABHA (Ayushman Bharat Health Account) card is the national health card, issued through Ayushman Bharat Digital Mission (ABDM) for seamless management and sharing of medical records. ABDM aims at creating an integrated digital health infrastructure in the country by encouraging everyone to create their ABHA ID in order to organize and maintain their medical history in a systematic way for better health tracking and progress monitoring. ABHA Number is a 14-digit identification number which allows users to share their health records digitally with hospitals, clinics, insurance providers, and others. ABHA Address (also known as Personal Health Records Address) is a self-declared username required to sign into Health Information Exchange & Consent Manager (HIE-CM). ABHA can be used to save your medical records and share them with the concerned doctors whenever required. The ABHA health locker secures your medical data and lets you access it anywhere anytime.
ABHA is applicable to everyone who is a citizen of India. When you create ABHA, it automatically gets linked to your Aadhaar card.
ABHA card allows the organization and maintenance of personal health records to ensure better health tracking and monitoring of progress. It enables seamless sharing through a consent pin to simplify consultation-related communication between patients and medical professionals. It has enhanced security and encryption mechanisms along with easy opt-in and opt-out features To create an ABHA ID, you need:
- Aadhar Card linked with mobile number OR
- Driving License (NOTE: You can initiate request for ABHA Number using DL, however you will only obtain an Enrolment Number. You will be required to go the nearest ABDM participating facility and verify your identity to obtain ABHA Number. Therefore it is recommended to create ABHA Number using Aadhar Card.)
Here is a step-by-step guide to create your ABHA ID via:

- ABHA PHR App
METHOD 1: Via the ABHA Web App
Step 1: Visit the ABHA website
- Visit the official ABHA website at https://healthid.ndhm.gov.in/register
- Select “Using Aadhar” option and Click on the “Next” button
Step 2: Submit Aadhar Number & OTP:
- Enter your 12-digit Aadhar Number in the highlighted text box.
- Check the “I Agree” option.
- Enter the captcha & Click on the ‘Next’ button.
- Enter the 6-digit OTP received on your registered mobile number linked with Aadhar.
Step 3: Verify Profile Completion & Download ABHA Card:
- Verify the auto-generated 14-digit ABHA Number & ABHA address.
- Click on “Download ABHA Card” and save the file.
METHOD 2: Via the ABHA PHR Mobile App
Step 1: Download and Install the ABHA PHR App
- From your mobile browser and visit the app download link
- Install the official ABHA PHR App. (NOTE: “Install from other sources” must be enabled in Settings to allow installation of outside apps.)
Step 2: Submit Aadhar Credentials and Create ABHA Number:
- Click on “Create Now” on the login screen.
- Select “Create your ABHA Number” and Click on “Continue”.
- Input your 12-digit Aadhar Number & check the “I Agree” box.
- Click on the ”Next” button.
- Select the validation option “OTP on Mobile Number linked to Aadhar”.
- Enter the 6-digit OTP received on your registered mobile number linked with Aadhar.
- Click on the ”Next” button.
- Enter the 10-digit mobile number linked for ABHA communication purposes.
- Click on the ”Next” button.
Step 3: Create ABHA Address & Verify Profile Details:
- Input your self-declared ABHA Address (eg. rohitsingh@sbx)
- Verify all other profile details & Click on the ”Next” button
- Click on “Download Card” and save the file.
